Please download, print and fill out the New Patient Intake Form and bring it with you to your first visit. There is a map on the last page to help you find our clinic.

This form is a very important diagnostic procedure for assessing your health so please take the time to thoroughly complete it.

Karen will review your information in depth and discuss your health concerns with you. She will be happy to answer any questions you may have about acupuncture and your condition.
